Pleas send entry fee along with the registration form. Also note that your team is not confirmed in the tournament until we have received full payment, registration form and waiver form. Make certified form of payment to SLAM DUNK ENTERTAINMENT and mail to: Attention: Clyde Pete – 1892 Felix Avenue – Memphis, TN. 38114. A late fee of $50.00 will be assessed on all registrations that are not completed and submitted by the deadline of the tournament. Please note: NO REFUNDS WILL ISSUED IF YOU CANCEL YOUR REGISTRATION Coaches are responsible for knowing all rules and regulations.
